Fix for AX 2012 R2 In-Place Upgrade: Database setup error “An item with the same key has already been added”

Sometimes during an AX 2012 R2 In-Place upgrade, when running the Setup for Database components to split the model store into a new database and apply the new R2 models; you may get the following error “An item with the same key has already been added”.
