Support recently ran into the error below when using the reverse engineering tool in AX 2012 R2. This problem will exist in AX 2012 too. To reproduce this error we also had to install Visio 2013. The error is was "Method 'add' in COM object of class 'IVDocuments' returned error code 0x86DB09C6 (<unknown>) which means: File not found."
